Based on the data collected, the T0 unique drop rates are approximately 20 times rarer than T1 items:
400 T0 unique items were estimated to have dropped across all players, compared to 1450 T1 uniques of each type.
In 200 maps, only one T0 unique was dropped using a setup with 0% item quantity gear.
Strategies to Target Farm T0 Uniques in Path of Exile 3.23
To increase the chances of farming T0 uniques, the following strategy was used:
The increased item rarity gained through the Affliction mechanic in Path of Exile 3.23 could potentially lead to players finding extremely rare and powerful unique items, including Tier 0 uniques. By heavily investing in blue wisps and mechanics that amplify item rarity, the article suggests some groups may be able to reach such high rarity multipliers of 300% or more applied to monsters. At those levels, it speculates the Affliction mechanic could create chances for T0 and T1 uniques to drop, which are almost never found under normal circumstances.
Use of Quantity and Rarity Gear: Employ gear that increases the quantity and rarity of items found. In the test, gear with 61% quantity and 500% rarity was used.

Tormented Spirits: These spirits increase the quantity and rarity of drops from affected mobs.
Rogue Exiles: Rogue Exiles drop more items, especially when they are tormented.
Altars in Maps: These can duplicate unique drops and offer additional quantity bonuses.
Selective Filtering: Use a strict loot filter to avoid low-value uniques, focusing on the potentially high-value T0 drops.
Additional Map Content: Consider adding other map content like Beyond, Beasts, Abyss, Shrines, or Alva to further increase the potential for high-value drops.
By applying these methods, players can optimize their chances of obtaining T0 uniques, albeit with an acknowledgment of the inherent volatility and rarity associated with such high-level drops.
